Lindsay Lohan checked into Promises Treatment Center in Malibu, California, on Monday afternoon, Usmagazine.com has learned.

A rep for Lohan tells Us: "Lindsay admitted herself to an intensive medical rehabilitation facility on Memorial Day. Because this is a medical matter, it is our hope that the press will appreciate the seriousness of the situation and respect the privacy of Lindsay as well as the other patients receiving treatment at the facility. There will be no other official comments regarding this issue."

The actress’ lawyer, Blair Berk, was seen driving Lohan (who was sporting a dark fedora), in a Mercedes at around 4:30 p.m. yesterday to the same facility that Britney Spears checked out of in March.

Sources tell TMZ this is "a serious medical treatment program," instead of the laissez-faire treatment she enjoyed during her stint at the Wonderland Center last January.

Her entry into rehab, her second stint in the last five months, comes after a whirlwind weekend of drinking, drug use and possible jail time.

Lohan’s wild Memorial Day holiday weekend started on Friday, May 25. At 11 p.m., she was chauffeured by her bodyguard, Jazz, to Hollywood hot spot, Les Deux, where she was spotted swigging vodka and dancing with sometimes pal, Paris Hilton, 26.

Three hours later, Lohan, her deejay pal Samantha Ronson, and a male friend headed out into the night, with brief stops at a party in Hollywood Hills and the Mondrian Hotel’s Skybar, where, says a club source, “Lindsay was a wreck.”

The three then headed towards Lohan’s West Hollywood home where the party took a dark turn. According to an eyewitness, Lohan and Ronson got into a heated argument.

“They were screaming at each other,” says the onlooker, who saw Lohan jump out of the SUV and yell, “You f—king lesbian bitch!” then run into her house. She emerged moments later, driving her Mercedes, and ordered Ronson into her car.

Along with the male friend, they took off and crashed a few blocks away. “All of a sudden, her car was between two palm trees,” says a witness, who saw Lohan walking around after the accident.

“She wasn’t hurt. She wasn’t bloody.” Still, 10 minutes after the accident, the actress called her bodyguard to pick them up and take them to the hospital, leaving the scene of the accident before the police arrived.

After an eye witnesses called 911, police caught up with the 20-year-old actress at the hospital and arrested her. As for the narcotics, cops say they found “a usable amount” of a drug tentatively identified as cocaine for which Lohan has yet to be charged.

On Sunday, the Georgia Rule star was at it again, dancing with Ronson at Hollywood hangout Teddy’s until 4 a.m.

According to the New York Post, when Lohan left the club, she stumbled through the back door and dropped her sunglasses and cigarettes. She bent to pick them up and collapsed, a witness said. A bouncer scooped her off the floor by her waist, and placed her in the passenger seat of the car, which had three plastic medallions from rehab treatment, each indicating 30 days of sobriety, hanging from the rearview mirror.

"She couldn't stand up anymore," said one witness. "She couldn't even move when she got in the car.”

Soon after, Ronson stopped at a gas station on Beverly Boulevard and tried to rouse her friend and alert her to the paparazzi snapping her pictures. But Lohan, whose eyes were closed and mouth hanging open, was unresponsive.

“Samantha was poking her,” says the onlooker. “She didn't respond. She was totally passed out."

The two then headed to an unidentified residence where they both spent the night. Lohan entered the treatment facility the following morning.

Says a pal, “She’s a troubled girl. Maybe this will be her wake-up call, but at this point, we don’t know.”
